How to schedule an intake appointment:

1. Please use our Online Intake Form to fill out your demographic details and fill out all required fields with a *red asterisk in order for us to create your patient portal for online scheduling. 

2. Please include your insurance info and a photo of your insurance ID. If self pay, please put this in the Insurance ID section. 

3. Putting a credit card on file is mandatory so that we can process payments of any co-pay, deductibles, or self-pay services. We will verify with you before charging any due balance. 

4. Once your intake form is received, we will review your info within 24-48 hours and send you an email to activate your patient portal if we can accept you as a patient.

5. Please activate your patient portal and bookmark the portal webpage, as it can be used to fill out intake and consent forms, schedule an appointment, pay bills online, and share or receive any documents or your viewable notes.

Once your portal is activated, you can use the above link at any time to log in or reset your password.
